Council of Europe Resolution on the implementation of pharmaceutical care – A step forward in the promotion of appropriate use of medicines and patient-centred care


Inappropriate and irrational use of medicines is a widespread problem which leads to sub-optimal medication outcomes and significant health harm for patients, and decreases the efficiency and effectiveness of healthcare systems.

Pharmaceutical care addresses the medication needs of patients directly and comprehensively and it contributes to promoting patient-centred care, optimising medication use and improving a patient’s quality of life.

This webinar aims to explain the content and added value of the above resolution, share examples of how pharmaceutical care activities can be implemented in daily practice in community and hospital pharmacy settings, provide insights about interprofessional cooperation in pharmaceutical care, and give an overview of the implementation of pharmaceutical care in some Council of Europe member states.
